Booking/Scheduling for Sacred Prana

1.“Consent and Policies” form is Liability release and policies information.

2. “LOCATION” contract : with most updated information and directions. Square text will also provide a basic map- but you will need to read the directions.

IF YOU ARE BOOKING MORE THAN ONE PERSON, EACH NEEDS TO BE WITH THIER NAME, PHONE & EMAIL. (unless this is a surprise gift then please let me know.) Everyone needs to complete a liability release- digital is preferred.
